The PCP Titration Handoff Model
Many PCPs are comfortable maintaining stable psychiatric medications but prefer a specialist for the initial evaluation, diagnosis, and stabilization phase. This is sensible — psychiatric medication initiation often requires more frequent follow-up, dose adjustments, side effect monitoring, and diagnostic refinement than a standard office visit allows. The PCP titration handoff model distributes this work according to each provider's expertise and capacity.
We do the heavy lifting: evaluate the patient, make a diagnosis, start or adjust medication, titrate to therapeutic dose, monitor for side effects and drug interactions, manage initial adherence challenges, and stabilize the patient. Once stable — typically 6–12 weeks depending on the medication and patient factors — ongoing management returns to you. You get the patient back on a clear, stable regimen with documented dosing, titration history, side effect profile, and monitoring schedule. No guesswork.
The patient stays on your panel throughout. There's no loss of continuity from the patient's perspective. They see us for a focused medication evaluation and stabilization, then return to your care for long-term management. You maintain the relationship and the primary care coordination. We handle the specialist work and report back with clear documentation.
