What We Do

Psychiatric medication management is ongoing care for patients who need a psychiatrist or psychiatric nurse practitioner to manage their mental health medications. If you're on psychiatric medications and need a specialist provider — either because your PCP wants to hand off care, because your previous provider is no longer available, or because you need a fresh evaluation — this is the service for you.

A common scenario: your primary care doctor has been managing your depression, anxiety, ADHD, bipolar disorder, or PTSD medications, but they feel it's time for a psychiatric specialist to take over. Another common scenario: you were seeing a psychiatrist or psychiatric NP elsewhere, but they've left their practice, retired, or you've moved to Alaska and need continuity of care. We evaluate your current medications, treatment history, what's working and what isn't, and any side effects you're experiencing. From there, we work together to adjust medications if needed — based on how you're responding clinically, any side effects, and your goals for treatment.

This is not a page about getting new prescriptions. This is about careful, individualized medication management. We review your complete history, understand what you've already tried, and make thoughtful adjustments based on your clinical response and life circumstances. Follow-up visits monitor how you're doing, screen for side effects, and ensure your treatment plan stays on track. We also coordinate with your PCP, therapist, and any other providers involved in your care, so everyone is working toward the same goals.

PCP Handoff
Your primary care doctor has been managing your psychiatric medications and wants you to see a specialist. We'll evaluate and take over from there.
Provider Transition
Your previous psychiatrist or psychiatric NP left their practice, retired, or you moved. You need a new provider for continuity of care on your current medications.
Medication Review
You're on multiple psychiatric medications and want a specialist to review the whole picture. Are these the right medications? Are the doses right? Are there side effects we can address?
Stabilization and Return
You need psychiatric evaluation and stabilization on new medications, then ongoing management can return to your PCP once things are stable.

The Process

What to Expect

Your initial visit focuses on a thorough review. We'll talk about your current medications — what they are, the doses, how long you've been on them, what they're helping with, and any side effects you've noticed. We'll explore your treatment history: what have you tried in the past? What worked? What didn't, and why? We'll assess your current symptom status, how you're functioning, and what your treatment goals are. This visit usually takes 60 minutes.

After the initial visit, we'll typically schedule a follow-up in 2–4 weeks to discuss any changes we've made and how you're responding. In this visit, we'll assess your response to medication adjustments, screen for emerging side effects, and refine the plan if needed. If your treatment is stable, future visits might be monthly or every other month. If we're actively adjusting medications, visits may be more frequent — sometimes weekly or biweekly — depending on what's happening clinically.

Throughout your care, we coordinate with your PCP, therapist, and other providers. Your PCP can request collaboration notes or speak directly with us about your treatment. The goal is seamless, coordinated care that works for you and your whole medical team.

Yes. We can evaluate your current medications and treatment history, and provide continuity of care. We'll review what's working, what isn't, and adjust the plan as needed. There is no need to discontinue your medications before your first appointment.
No. You can schedule directly. A PCP referral is helpful context but not required. Many patients come to us because their PCP has referred them for specialist medication management.
Visit frequency depends on your clinical needs. During medication adjustments or crises, visits may be weekly or biweekly. Once stabilized, visits may be monthly or every other month. Your provider will discuss the plan with you.

Absolutely. We actively coordinate with your PCP, therapist, and other providers. This ensures you have a cohesive treatment plan and that your prescribing providers are on the same page. Your PCP and other providers can request collaboration notes.
That's an important conversation we'll have together. We never recommend stopping or reducing medication suddenly without clinical guidance. Depending on the medication and your symptoms, we'll work out a safe tapering plan and monitor for any changes in how you feel.
